A beautiful song, and he has one of my favourite male singing voices.

I like how fame and love are equated, giving some hint as to the nature of the other person in the relationship.

Especially interesting is the first two lines. Hope by itself is not a personification, so those lines are an equation. But taken into context with the rest of the song it seems to be saying the woman, whoever she is, is his hope. Yet the hope he has in her ends up being broken.

From what I can gather the poison in her arm is literal, which is what kills her, causing the writer of the song to have a "heart of darkness." I'm not sure what the sacrifice is, though.

After thinking about it a bit more, it seems that the person being sung about is a model of some renown, whose sacrifice for her career is the lifestyle required. This leads to a drug overdose and her death.
